Account Terms
The Card(s) is issued by a Credit Card, When your application is approved, the complete terms applicable to this account will be furnished to you with your Card(s).

Authorization: I certify that all information given in this application is true and correct and I am giving this information in order to obtain credit and authorize First National Credit Card ("the Bank") to obtain information concerning any statements herein. I agree that the Bank may report its credit experience on my account to and exchange information relative to my account with reporting agencies and/or others. I agree to furnish the Bank with all requested information. I authorize Credit Card to charge the Annual, Acceptance, Program, and Monthly Participation Fees to my account.

Refund/Cancellation: The annual fee, acceptance fee, program fee, and monthly participation fee are "membership fees" for truth in lending purposes. After we receive your application, you will be sent a cardholder agreement containing all of the terms applicable to your account. If you elect to close your account within 30 days of receiving your cardholder agreement and before you make any additional charges to the card (or any renewal card ), the Annual Fee, Acceptance Fee, Program Fee and Monthly Participation Fee will be refunded to you.

Easy VISA® Requirements: Your application is based upon the information you supply us on your application. You must be 18 years of age or older, not presently in bankruptcy and have a verifiable residence and proof of income (minimum $10,000 annually). Within the last six months you can not have had any major derogatory credit or any delinquencies of 60 days or more past due, and you must have one established positive credit bureau reference of at lease six months.

Qualify For Additional Credit: If you make all your payments on time for only six months and pay substantially more than the minimum due each month, you will be eligible to receive a credit line increase. Initially, you will be issued a VISA® with with not less than a $250.00 credit line up to a maximum of $1,500.00. This program is designed to allow individuals to increase their credit line by establishing creditworthiness with Credit Card.

Optional Payment Protection Plan:This optional program pays your minimum monthly account payment if you, the Primary Applicant, become totally disabled, involuntarily unemployed or take an unpaid leave of absence for more than 30 days. It also pays the balance on your account if you or your Insured Co-Applicant should die. Life and Disability benefits are payable up to the sum of your unpaid balance at the time your loss occurs or $5,000, whichever is less. Unemployment and Family Leave benefits are limited to 9 monthly benefit payments (3 monthly payments for the care of a newborn or adopted child). The cost is 75˘ per $100 of your ending monthly balance and is included on your billing statement each month you have a balance. Family Leave insurance covers unpaid absences from full-time employment while you or your Co-Applicant are: caring for an immediate family member with a serious medical condition, caring for a newborn or adopted child, on active military duty due to mandatory recall, on jury duty, or residing in a federally declared disaster area. Family Leave insurance does not cover losses caused by a pre-existing medical condition. A "pre-existing condition" is one for which you or your family member saw or was under treatment by a physician or chiropractor both within the 6 months before and the 6 months after the Effective Date of Coverage. Life insurance benefits are not payable in the event of suicide within 6 months of the Effective Date of coverage. Disability insurance does not cover losses caused by: normal pregnancy or childbirth; any intentionally self-inflicted injury; or a pre-existing medical condition. Your Co-Applicant is not eligible for disability or involuntary unemployment insurance. All coverage ends at age 66. In AL, GA, MI, MO, NM, NV, OK, VA and VT, coverage ends at age 71. In AZ, the Life insurance changes to Accidental Death insurance at age 70. In CO, OR, PA and SC, your plan includes only Life, Disability and Unemployment insurance at a cost of 59˘ per $100 of your ending monthly balance (58.7˘ in OR; 48˘ in PA). In VT, your plan includes only Life, Disability and Family Leave insurance at a cost of 40.9˘ per $100 of your ending monthly balance. In AL, CA, ME, NC, SD and VA Family Leave benefits are limited to 6 monthly benefit payments (3 months in AK, CT, HI, MA and NH) for each payable claim. In PA, only the Primary Applicant is covered for Life insurance. If you wish to purchase these coverages separately, call 1-800-445-6500. In TX, only you are covered for Life insurance and family leave coverage is not available; your adjusted cost is 42.7˘ per $100 of your ending monthly balance; to be eligible for Unemployment insurance, you must be employed at least 30 hours per week in a non-seasonal occupation for at least 90 consecutive days prior to the effective date of coverage (self-employed cardholders and independent contractors are not eligible for coverage); unemployment coverage excludes retirement, resignation, job loss due to willful or criminal misconduct and disability; and, if you wish to limit your coverage to Life and Disability only at a cost of 22.7˘, call Central States at 1-800-445-6500. In CO, to be eligible for this insurance, you must be employed at least 30 hours per week on the date you enroll. In IL, Unemployment benefits are not payable for unionized labor disputes, general strikes or lockouts. In MA, disability benefits begin on the 31st day after your date of loss. In MD, the Co-Applicant must be your spouse. Coverage is not available to residents of NY. In VA, special enrollment materials are required; please sign the enrollment and the appropriate materials will be sent to you. This program is offered, administered and underwritten by Central States Indemnity Co. of Omaha and also underwritten by Central States Health & Life Co. of Omaha, Nebraska. Each applicant may be liable for amounts extended under the plan to any joint applicant. Wisconsin Residents: Are not eligible for this program. Kentucky Residents: You may pay the unpaid balance of your account in whole or in part at any time. Maine Residents: Credit insurance provided herein is voluntary and your have the right to cancel such credit insurance at any time.
